Worker fell through a roof while working on a maintenance building

Pacific Rim Industrial Insulation Ltd. has been fined $210,000 after pleading guilty to an occupational health and safety violation that resulted in the death of an employee at a production facility north of Fort McMurray, Alberta, in June 2023.
Court documents show that Milo Sagmoen, an employee of Pacific Rim Industrial Insulation Ltd., died after falling through a roof while working on a maintenance building at Chemtrade’s ammonium sulphate production plant within Syncrude’s Mildred Lake site. The incident occurred during contracted work at the facility, which is not affiliated with Pacific Rim Industrial Insulation Ltd., according to a Chemtrade representative who spoke with CBC News.
On Monday, Pacific Rim Industrial Insulation Ltd. was convicted of failing to ensure a worker used a travel restraint system where there was a risk of falling three metres or more. The company pleaded guilty to the charge and was ordered to pay the fine and undergo 18 months of enhanced regulatory supervision. Three other occupational health and safety charges related to the incident were withdrawn .
Under Alberta’s occupational health and safety legislation, employers are required to enforce the use of fall protection systems for employees at risk and to ensure workers are informed of all fall hazards. CBC News said it reached out to Pacific Rim Industrial Insulation Ltd. for comment on the conviction.
